Government Rebate for Eligible Patients
At Head 2 Toe Solutions, we accept Medicare Chronic Disease Management (CDM) plans (formerly known as Enhanced Primary Care or EPC plans) for physiotherapy treatment. If you have a long-term condition affecting your movement and function, you may be eligible for Medicare-subsidized physiotherapy sessions under this plan.
What Is a Medicare Chronic Disease Management Plan?
A Chronic Disease Management (CDM) plan, previously called an EPC plan, is a referral from your GP that allows eligible patients to access up to five Medicare-subsidized allied health treatments per calendar year.
This plan is designed to support patients with chronic conditions who require ongoing physiotherapy care to manage their condition, improve movement, and enhance daily function.
Am I Eligible for a CDM Plan?
Medicare defines a chronic condition as any issue that has lasted (or is expected to last) for six months or longer. This can include conditions such as:
✅ Arthritis & joint conditions
✅ Chronic back or neck issues
✅ Diabetes-related musculoskeletal concerns
✅ Post-stroke rehabilitation
✅ Chronic pain syndromes
✅ Osteoporosis & balance disorders
✅ Respiratory conditions affecting mobility
Your GP will determine if a CDM plan is suitable for you based on your condition and healthcare needs.
Medicare Rebates & Gap Fees
Under the CDM plan, Medicare provides a rebate for five physiotherapy sessions per year. At Head 2 Toe Solutions, we apply these rebates as follows:
💰 Standard Gap Fee – $38.70 per session
💰 Pensioner Discounted Gap Fee – $28.70 per session
Patients must pay the full session fee upfront, and the Medicare rebate of $58.30 per session will be processed on the same day.
Unfortunately, we do not offer bulk billing.
How to Access Medicare-Subsidized Physiotherapy
1️⃣ Visit Your GP – Ask if you are eligible for a CDM/EPC plan. If approved, your GP will prepare the referral.
2️⃣ Obtain a Referral to Head 2 Toe Solutions – Your GP must list our clinic and our physiotherapists as your providers.
3️⃣ Book Your Appointment – Call us or book online, and bring your referral to your first session.
4️⃣ Claim Your Medicare Rebate – Pay upfront, and we will process your $58.30 rebate directly through Medicare.
